In addition to completing the subcontractor application, please have the following supporting documentation to upload when asked before starting.
- Insurance certificates with all applicable endorsements
- Individual insurance coverages along with the limits of coverage & expiration dates on each policy
- OSHA forms 300A and 300 (omit employees names) for the past three years
- EMR verification for the past four years (NCCI/comparable form or letter from workers compensation provider)
